Taking Charge® A Transition Planning Partnership:

Workbook I: My Life After High School

What do I want my life to look like after high school? What is my day going to look life after high school? Do I want additional training or education? Do I want to work? Do I want to join the military? Where will I live (at home, with relatives, with friends, by myself)? What will I do to exercise? What will I do to have fun? Will I have any pets? What will I do in my community? Do I want to be a volunteer? How will I pay for my clothes, food, car, and car insurance? How do I create a daily or weekly schedule for myself?

Workbook II: My Concerns and Goals

What are my concerns? What are my choices and their consequences? How do I turn my concerns into goals? What are the barriers to me reaching my goals? How can I reach my goals despite my barriers? Who can support me?

Workbook III: My Learning Competencies

What competencies do I now have that can help me reach my goals? What competencies do I need to gain to reach my goals? What steps do I need to take today, tomorrow, next week, next month, this year? Who can support me?

Workbook IV: My Success Strategies

How can I build a network of support? Who can I talk to about my concerns/goals? Who do I support? What if people let me down? How can I make positive choices? How can I remember to take small steps to reach my goals? How can I continue to set new goals? What people can support me? What organizations or agencies can support me?

Workbook V: My Action Plan

Here is my action plan, a summary of what I need to know and learn; strategies I already have and strategies I need to develop; and the steps I need to take today, next week, next month, this year. Here is a summary of my requests for support from people, organizations, or agencies.
